We are required to provide a signed Waiver of Liability to the St. Albert Curling Club from each curling member this season.  Please print, complete the Waiver by initialling in each of the 2 'boxes' and Printing your Full Name with your signature and date at the bottom.  
Return the completed form to the office at the Curling Club before your next game.
